Overview

(01)

A life of your choice, with the support to get there.

Your Support Coordinator helps you understand your options, plan your services and speak up for what you need.

We work with you, the people you choose to involve and your service providers. Together, we develop your plan and check that the support you receive meets your needs.

That support also looks beyond services, helping you build relationships and take part in your community.

Building a close and trusted relationship with the individual and their support network sets apart Support Coordination from typical case management services.

Your journey with PCSA, step by step.

What happens, what to expect, and what you can do at each step.

  1. Step 1

    Reach out to us

Call our Valdosta or Tucker office, or use the contact guide to prepare an email. Tell us a little about yourself or your loved one.

Tip: Have your Medicaid information nearby if you have it.

  2. Step 2

    Choose PCSA

When you are approved for a Georgia NOW or COMP waiver, you'll be asked to choose a Support Coordination agency. You have the right to choose, and you can choose PCSA.

Tip: Not on a waiver yet? Find your DBHDD regional field office on our Locations page.

  3. Step 3

    Meet your Support Coordinator

Your Support Coordinator gets to know you, your family and the people who support you. This is someone you can call with questions.

Tip: Save your coordinator's phone number in your phone.

  4. Step 4

    Create your plan together

Together we develop your Individual Service Plan (ISP), setting out your goals, the support you need and who will provide it.

Tip: Before the meeting, write down 2 or 3 things you'd like to do or change in your life.

  5. Step 5

    Your services begin

Your Support Coordinator interacts with your providers to make sure your individual needs are being met.

Tip: It's okay to ask a provider questions before you decide.

  6. Step 6

    Ongoing support

We monitor your ongoing needs, ensure the quality of your services, and work to maximize your health and safety.

Tip: Something not working? Tell your coordinator right away.

Questions

Frequently asked questions

Don't see your question? Our team is happy to help.

Can a family member help me get started?

Yes. A family member or caregiver can contact us with questions or help you prepare a request. Your own preferences and goals remain at the center of planning.

Does the contact guide send my message automatically?

No. It prepares a draft in your email app. Review the message and press Send there. If no email app opens, you can copy the message and send it yourself, or call either office.

What is an ISP?

ISP stands for Individual Service Plan. It's your written plan: your goals, what's important to you, and the services and supports that will help. It's person-centered, which means it's built around you.

How do I apply for a Georgia waiver?

Waivers are managed by the Georgia Department of Behavioral Health and Developmental Disabilities (DBHDD). Start by contacting the DBHDD regional field office for your area. You'll find every field office on our Locations page, and more help on our Resources page.

Where are your offices?

Our main office is at 2404 Bemiss Road in Valdosta, and our metro office is at 3543 Habersham at Northlake Rd, Bldg. E in Tucker. See maps & directions.
